Referral Forms and Traumatic Injury Treatment Guidelines
For your convenience, you can place these documents on your desktop. Right click on the form you would like to save. Please choose/highlight the "Save link as" option and left click your mouse. A pop up "save as" screen will appear. Make sure the top tool bar has desktop listed. You will just need to click on the save button at the lower right hand corner of the screen and it will create a PDF file on your desktop of the document you chose to save. You can print our referral forms directly from your desktop.
